Palliative and End of Life care

You may choose to work on our team providing Palliative and End of Life Care, providing emotional support as well as practical care, such as assistance with personal care, household tasks and medication needs. You will be supported and mentored by our clinical case manager who will oversee any clinical tasks which may be required.

Although End of Life care typically supports people living with advance or progressive conditions this isn’t always the case. End of Life can be long or short term and there are different care options available that can accommodate an individual’s needs and wishes. You will be joining a team who is passionate about ensuring an individual receives respectful, dignified care and support by taking a holistic approach.

You will liaise with healthcare professionals including GPs, district nurses and specialist palliative care nurses to make sure an individual is as comfortable as possible. Palliative and End of Life care can be delivered in a bespoke flexible manner that suits a person’s needs.
